3, 加收款人 add "payee".

5, 輸入教會地址 8705 Lindell Road #150, Las Vegas, NV, 89139 及電話號碼: 702-516-5126.
6, 備註或註釋: (你的英文全名"full name", 用於報稅用途).
7, 日期: 帳單付款 Billpay 需要五個工作日讓銀行寄出支票至本會.
註： Billpay並不是銀行現金支票 Money Order 或 Cashier Check.

3, Add "Payee".
